An air declaration form is mandatory for all air shipments of hazardous materials / dangerous goods.
The IATA Shipper’s Declaration is used when shipping hazardous materials and dangerous goods by air. It contains information such as the shipper’s name and address, the cosignee’s name and address, the departure and destination airports, dangerous goods identification, proper shipping name, class or division, UN or ID number, subsidiary risk, quantity, type of packaging, and additional handling information.
These air declaration forms meet the requirement of IATA, section 8. All shipments of dangerous goods require the use of an air declaration form that must be signed by a trained person. ICC The Compliance Center provides these forms 8.5″ x 11″ (21.59 cm x 27.94 cm), in packages of 100. Available in two styles: 4-part snap set NCR – blank or customized, or laser sheet. The laser sheet is available in two styles. Available English only or bilingual – English and French.
